Records behind the service total
Each completed service contributes once. These totals measure recorded activity. Clinical outcomes are assessed separately.
In this example, recording the third service updates the report from 2 services to 3.
Connected by the program record
Bring participant information, required items and review status into one program record.
What it gives you
A clear view of who is enrolled, what is missing and who is ready for the next step.
Give each next action an owner and a due date, then document its completion.
What it gives you
A shared view of open work, completed services and referrals that need attention.
Define the measures and reporting period, then build totals from recorded activity.
What it gives you
A report staff can trace to the records behind each number.
Different programs, the same core workflow
Review enrollment, assign appointment follow-up and record delivered services for the program report.
Organize sign-ups, assign outreach actions and report documented participation over a defined period.
Record referral requests, assign follow-up and report the status of each documented handoff.
Fit with your existing systems
Clinic Program Management is intended for program administration and reporting where an additional workflow is needed. Clinical records and clinical decisions stay with your clinical team and systems.
Before implementation, we map the information already recorded, identify any duplicate entry and agree how data can move between systems. Imports and integrations depend on your systems and must be confirmed in the proposal.
The workflow must justify the staff effort it adds. If the EHR already covers the program, that is the starting point.
Implementation and ownership
Agree the enrollment fields, review stages, task owners, permissions and reporting measures.
Your program lead supplies sample records, current forms and reporting requirements. Together, we agree which system owns each field.
Reconcile sample reports to source records. The clinic’s program owner approves the definitions and workflow before use.
Walk staff through enrollment, follow-up and reporting, with written guidance, named owners and an agreed support process.
Your proposal separates one-time implementation, ongoing software and support costs. Pricing follows the agreed program scope, data mapping and support needs. Changes to scope are agreed before work starts.
Bring your current workflow, reporting requirements and the follow-up that is hardest to keep moving.
